Proxy Statement For The National Council on Independent Living 2011 Annual Council Meeting
We (I) are (am) unable to participate in the NCIL Annual Council Meeting to be held on Saturday, July 16, 2011 at the Grand Hyatt Hotel in Washington, DC. We (I) are (am) therefore appointing the following individual to present our (my) proxy vote in our (my) name, place, and stead at the Annual Council Meeting and at any adjournment thereof, as fully as if we (I) were (was) personally present and represented. Our (My) proxy is for the election of NCIL officers and Member at-large positions and for any other issue which is called to a vote of the NCIL membership at the aforesaid Annual Council Meeting.
Please note: CIL, SILC and Organizational Members may only designate one individual to act as their representative at the Council Meeting.
Proxies must be received by Friday, June 24, 2011 by 5:00 p.m. EDT.
Proxies not sent by June 24, 2011 may also be handed in at the membership table at the NCIL Conference. ALL PROXIES MUST BE RECEIVED AND ALL VOTING CARDS PICKED-UP BY THE COMMENCEMENT OF THE ANNUAL COUNCIL MEETING, BY 2:00 P.M. EDT ON SATURDAY, JULY 16, 2011.
