Find out how you and your staff can assist consumers to identify, learn (or re-learn) the skills that will increase the chances of a successful transition – indeed, you’re already doing it!

It can be difficult and time consuming to assist consumers moving back into the community from nursing homes and other institutions.  But don’t let that stop you from doing it!  The transition process is little more than a collection of the four core services tailored to the goals of each consumer.   That means that independent living skills are an integral piece of almost every transition. Our expert presenters will explain how to apply what you already know and how to expand your toolbox of IL skills offerings to include individuals who are transitioning.

Bruce Darling
Rochester Center for Disability Rights

Bruce Darling is co-founder and Executive Director of the Center for Disability Rights (CDR), a Rochester-based disability rights organization and Independent Living Center.  During the summer of 2000, without any new funding but a great deal of conviction, CDR began a project to transition people out of nursing homes and back into the community.  Since that time, the project has helped hundreds of people return to community living, and CDR has hired staff dedicated to this transition work.  Bruce and the project staff have worked with many other community groups and Independent Living Centers to teach them about the Olmstead decision and to begin nursing facility transition projects of their own.

Jane Schiele
Disability Center for Independent Living

Jane Schiele has worked at Independent Living Centers since 2000.  She was the Statewide Coordinator for the grant-funded Nursing Facilities Transition Project in Colorado from 2002 to 2004.  The project was so successful that it is now permanently in Colorado’s legislation and is named the Community Transition Services. Currently, Jane works as the Director of Satellite Offices for the Disability Center for Independent Living in Denver, Colorado.  On the national level, Jane has presented at the NCIL Conference in 2006 and at the ILRU conference on Nursing Home Transition in 2004.  She was a director of the DVD, “You Have A Choice”, distributed through the ILRU. She also authored a Nursing Home Transition Project Handbook. She holds a Masters Degree in Social Work from the University of Denver in Colorado. 

Target audience:

The target audience members for this training are Executive Directors, Directors of Independent Living Services, and direct service staff of Centers for Independent Living who are new to the transition process.  .  

Learning Objectives:

Upon completion of this webcast, participants will have knowledge and resources that will aid them to:

  • Describe how IL philosophy and CIL staff support a successful transition process by reinforcing the peer-to-peer approach
  • Identify skills and services most often needed by individuals who are transitioning out of an institution and into the community 
  • Explain the process for assessing an individual’s training needs prior to and after the transition
  • Assist individuals to develop a personalized training plan to ensure their successful transition and integration into community living
