Target Audience:
The primary target audience for this introductory training is IL personnel who are NEW to nursing home transition. Transition facilitators, independent living specialists, program coordinators, and other core IL staff who work with consumers who wish to move out of nursing homes may also find this course helpful in understanding the issues of nursing home transition.
Training Objectives:
Through participation in this webcast, you will be able to:
- Describe strategies for locating and making the connection with individuals who want to transition out of nursing homes;
- Explain how to build rapport with consumers for productive and effective interactions;
- Describe the importance of the initial assessment of the individual’s needs and the critical role of planning to ensure success; and,
- Identify strategies for building good working relationships with institutional staff, family members, and others who either support or challenge the transition.
Your Presenter: Judy Roy
Judy Roy is Project Coordinator for Partnerships to Independence, a nursing home transition project of Independent Living Resources of Greater Birmingham. Judy is the PAS Subcommittee chair for the National Council on Independent Living. She is the Chair of the Alabama Long Term Care Coalition and contact for the ADAPT Chapter of Alabama. Judy serves on the Board of Alabama Arise, a coalition of 132 religious, community, and civic groups that promote state policies to improve the lives of low-income people.
